Regimens are categorized according to the recommendations of the respective CCO Disease Site Group. Specific eligibility criteria for use of a particular regimen may be applied in some centres, but have not been included here -- with the exception of restricted CCO regimens. The categories are defined as follows: Core, Core-Not Funded, Core-Restricted CCO, Local, Emergent, and Excluded/Archived.

Note: It is the policy of the Formulary Team to refer to all drugs by their generic names, although available manufacturers and applicable trade names are referenced where applicable. However, some multi-drug regimens are commonly referred to by acronyms which may refer to the trade name of one or more component drugs (e.g. CAF : Cyclophosphamide, ADRIAMYCIN® (Doxorubicin), Fluorouracil). For ease of use, and to be consistent with practice, acronyms in common usage are displayed, with a text description using the generic name. ( CAF : cyclophosphamide, doxorubicin, fluorouracil / AC-Taxol : doxorubicin, cyclophosphamide, paclitaxel). This does not constitute endorsement by the Formulary of any particular brand of drug in circumstance where multiple generic equivalents are available.
