Why Did we Create the Quality Improvement Plan Toolkit?
Ontario's Action Plan for Health Care highlights a focus on quality in primary care. An essential element of Ontario's healthcare transformation agenda is the introduction of Quality Improvement Plans (QIPs) to the primary healthcare sector.
To support this, we have developed two quality improvement toolkits for primary care.
