Screening Activity Report

The Screening Activity Report (SAR) provides patient enrolment model (PEM) primary care physicians with a supplementary tool for improving their cancer screening rates and appropriate follow-up for breast, cervical and colorectal cancer screening.

Note: Once you click on the ‘Access Your SAR’ button, you will be taken to an updated Cancer Care Ontario login page. Please click on ‘ONE ID’ in order to proceed with the login process (previously called ‘SAR Login’).

News

• Your SAR will now be updated monthly! Starting December 10th, 2014, the information in your report will be updated on the 10th calendar day every month to provide more timely breast, cervical and colorectal cancer screening data.
• If you are accessing this data to perform comprehensive tasks, please export and save your SAR to your computer. Your old report will always be replaced by the most recent version.
• What’s new in the report?
  • The SAR platform has been enhanced so that the reports are interactive. You can quickly drill-down to the specific cancer screening information you are looking for.
  • You are still able to export your reports to a PDF or excel. Please take a look at our short, instructional videos below to learn more about how to navigate your SAR.

The information contained in the SAR is highly sensitive and contains Personal Health Information (PHI). The disclosure of PHI to family physicians is permitted under Personal Health Information Protection Act (PHIPA), 2004.
